Columbia County Homeless Services
Operating since 2012, our Columbia County Homeless & Housing Services program is a FREE, three-phase, 12-month program that provides care, case management based on the Critical Time Intervention model, and a trauma-informed approach to Columbia County residents who are currently unhoused and in need of critical support services.
This program is proven to help individuals and families transition back into a stable life in the community after experiencing homelessness. This is a referral-based program and all clients must be referred to the program by the Columbia County Department of Social Services.
About the Program
Services Provided
- Time-limited case management model
- Work with case managers on two or three concrete goals helping clients get housing, employment, etc.
- Help in building a network of supports
- Formal: programs and other agencies
- Informal: friends, family, churches, etc.
- Works toward independence from staff as time goes on
CTI vs Traditional Case Management
- CTI focuses on two to three goals while traditional services may overwhelm by trying to fix too many issues at once
- Most traditional services have no rules regarding length or intensity of the program; CTI is a three phase, 12 month system
- CTI helps clients build support networks to increase the chance of success
Three Phases of CTI
- Phase 1: Transition to Community – Months 1-4
- Begins on day of enrollment
- Assessment of support system, development of treatment plan
- Phase 2: “Try Out” – Months 5-8
- Linking clients to outside services, locating of gaps in services
- Phase 3: Transfer of Care – Months 9-12
- Keeping track of now-established support system, finalizing supports for the long-term
Galvan Civic Motel
Located on Route 9, near Livingston, the Glavan Civic Motel is owned by the Galvan Foundation. This site is contracted through Columbia County to provide emergency housing for our clients.
This facility has 24 rooms and is located less than six miles from Columbia County Social Services.
Program Eligibility
To be eligible for this program, individuals or families must be currently homeless in Columbia County. Individuals or families must be referred to the program by Columbia County Department of Social Services.
If you are currently housed, even in temporary housing, you are not eligible for this program.
