Parent/ Guardian Consent Forms
According to New York State law, all children entering Kindergarten, 1st, 3rd and 5th grade are required to submit an updated annual physical and dental certificate. The annual physical must be dated NO EARLIER than 9/5/23.
COVID-19 Testing Consent Form
Please take a moment to sign our consent form allowing COVID-19 testing for children. Your support in ensuring their health and safety is greatly appreciated.
Medication
Consent Form
If you would like your child to be able to receive over the counter medication at school, please download and sign the form below. Then, have your pediatrician sign it as well before returning it to the school nursing office.
Sunscreen

If you would like your child to be able to have sunscreen applied at school please download and sign the form below and return it to the school nursing office.
